IU Health South Central Region (SCR) is defined as the 11 county area surrounding and including Monroe County.  SCR Physicians report 11% of their patients suffer from Type 2 Diabetes (DM2), however diabetes education differs by site. Using Simpler/Lean Tools, a method used to define potential waste in terms of time and cost to better improve patient care, IU Health Bloomington and SCR Physicians developed a team approach to treating DM2 consistently across the SCR. The focus of this project is to determine the client’s motivation to attend this integrated service and complete the program.
